When urticaria and angioedema involve the ears, pinna is typically erythematous and swollen due to dermal or subcutaneous edema (Fig. 10.13). These patients may complain of severe pruritus. While urticarial plaques resolve in a short time, new lesions may occur on other parts of the body. Cellulitis may present with a tender erythematous patch associated with edema. Auricular involvement is mostly unilateral in this bacterial infection which may develop due to ear piercing. Patients also have systemic symptoms such as fever and chills. It may start from the ears and spread towards the surrounding skin. Sometimes cellulitis may develop in the setting of seborrheic dermatitis. Systemic antibiotics are the mainstay of the treatment. As a result of the injury in the lymphatics of the external ear, recurrent attacks of cellulitis may develop on the ears and lead to thickening of the overlying skin.
